All incoming freshman must complete the entire medical information packet and physical examination prior to practice or competition. The physical examination will be administered by our team physician once the student has arrived to campus. Returning varsity athletes must complete an updated medical history and insurance information.

Please note that the physical examination process for varsity athletes is entirely different then the physical examination needed to be a full-time resident student at Waynesburg University. Our athletic physical examiniation will be conducted by our Team Physican to clear the athlete for intercollegiate athletics. A physical exam by a personal physician will not be accepted. Once you have downloaded and completed the appropriate forms, please mail these forms to Barb Tuggle (Athletics Admistrative Assistant) in the Athletics Department at Waynesburg University. The address is at the bottom of the page. All medical information must be received by the Athletics Department no later than July 1st, 2010.
