Medical Eligibility

Medical policies and procedures have been established to clarify the process of health care for student athletes participating in intercollegiate sports at Waynesburg University. It is imperative that the policies and procedures be followed to insure quality health care for student athletes. There must be a collaborative effort on the part of the Athletic Training Staff, the Athletic Department, and the student athlete to maintain an effective and successful health care program.

 

Physical Examination:

All incoming freshman and Transfer student athletes must pass a pre-participation physical examination and orthopedic screening before participating in intercollegiate sport activities at Waynesburg University. This physical examination is separate from the physical examination required to be a student at Waynesburg University

 

Student athletes that become injured or ill anytime during the school year will have to be cleared by the Athletic Training Staff in conjunction with our Team Physican before participation can resume or another sport can be played.

 

Student athletes who do not pass the pre-participation physical examination and screening will not be permitted to participate in any physical activity until they are cleared by the athletic training staff in conjunction with the Team Physician.  Until such time, the head coach and staff will not permit student athletes under special restrictions to participate in any physical activity involved in the intercollegiate sport.

 

Participation Prerequisites

Medical History Questionnaire or Updated Medical History:
• Student athletes must complete and return to the athletic training staff a medical history questionnaire prior to participation each year.

• Failure to complete this form will disqualify the student athlete from participation until it is received.

• Student athletes that willfully withhold information pertaining to their health status will relieve Waynesburg University of all responsibilities should a subsequent injury, reinjury, or illness occur.

• All injuries sustained prior to the pre-participation physical examination must be reported.

• All known diseases must be reported.

• Loss of a paired organ (ie. eye, kidney, testicle, etc.) must be reported.

• All illnesses must be reported.

 

Health Insurance Form Questionnaire:
• Student athletes must complete and return the insurance information form along with a photo copy of the front an back of the insurance card

• Failure to complete this form or mail a photo copy of the insurance card will disqualify the student athlete from participation until it is received.

• Student athlete must produce evidence of primary coverage by means of their own insurance policy, their parents/guardian insurance policy, or their participation in Waynesburg University student insurance policy. Student athletes utilizing the Waynesburg University student insurance policy must indicate so on the Insurance Form Questionnaire.

• Student athletes not able to provide primary insurance coverage must indicate so on the Insurance Form Questionnaire.

 


Informed Consent for Medical Treatment:
• Student athletes must complete and return this form in order for the athletic training staff to provide medical care, treatment, and Rehabilitation.

• Failure to sign and return this form will disqualify the student athlete from participation until it is received.

 

Acknowledgement of Risk and Release of Liability Agreement
• Student athletes must read and sign this agreement prior to participation.    

• Failure to complete this form will disqualify the student athlete from participation until it is received.

 

HIPAA Form:
• Due to the concern over medical record confidentiality, it is necessary for the Athletic Training Staff at Waynesburg University to obtain your written authorization to gather and release your medical information.  By signing this form, the student athletes releases the athletic training staff at Waynesburg University of any and all legal responsibility or liability that may arise from this authorization. This authorization is good for 5 years of the date signed.

• Failure to complete this form will disqualify the student athlete from participation until it is received
